If you participate, you will pay a quarterly, out-of-pocket membership fee. NO discount for each additional family member. You can use HSA or FSA to pay this fee. The fee covers:

• THREE provider appointments per quarter, includes in-office or virtual.

• Guaranteed appointment within 24-hours (Mon-Thurs 8:00AM-5:00PM) - in-office or virtual.

• One pellet insertion or 12 testosterone injections quarterly.

• In-office courtesy lab draw fees and urine dips

• 25% discount on any IVF administration; Lipovite fat-burner injections; weight management products; and supplements

• 10% discount on peptides ordered through Tree of Life Medical.

PLAN B

If you participate, you will pay a quarterly, out-of-pocket membership fee. Twenty-five percent (25%) discount for each additional family member. You can use HSA or FSA to pay this fee. The fee covers:

• THREE provider appointments per quarter, includes in-office or virtual.

• Guaranteed appointment within 24-hours (Mon-Thurs 8:00AM-5:00PM) - in-office or virtual.

• One pellet insertion or 12 testosterone injections quarterly.

• Annual executive physical with EKG and annual screening labs (cholesterol panel, diabetes screening (HgA1c), complete metabolic panel (CMP) and complete blood count (CBC)).

• In-office courtesy lab draw fees and urine dips.

• Intravenous fluid (IVF) administration quarterly.

• Monthly Vitamin B12 injections.

Two of the following will be included:

• Free membership to MY247MD app/website which allows patient and entire household free 24-7 telemedicine services, allowing after-hours or weekend medical concerns to be addressed.

• Cash-pay hormone/thyroid/vitamin labs (lab panels used for hormone follow up appointments).

• Twenty-five (25%) percent discount on Lipovite injections, supplements, weight management programs/meds.

• Ten (10%) percent discount on peptides ordered through Tree of Life Medical.

• FIVE provider appointments FOR YOU OR FAMILY MEMBER per quarter, includes in-office or virtual.

• Guaranteed appointment within 24-hours (Mon-Thurs 8:00AM-5:00PM) - in-office or virtual.

• In-office courtesy lab draw fees and urine dips.

• 25% on any IVF administration; Lipovite fat-burner injections; weight management products; and supplements.

• 10% discount on peptides ordered through Tree of Life Medical.

‍This is NOT a program you must utilize.

If you do NOT participate, there is nothing you need to do at this time.

Arrive at your appointment as scheduled.

If you have insurance, pay your copay/deductible as usual.

If you do not have insurance, pay the usual cash-pay appointment fee.

You will be charged the usual fees for labs, pellets, injections, weight management, supplements, medications and services.

Choosing NOT to participate does NOT increase appointment wait times, timely scheduling or any aspect of your care.
